Billing & Insurance Information

If you need a prescription refilled, please have your pharmacy fax a refill request to our office at (626) 449-0925. We do not refill prescriptions online.

We accept the following insurances:

  • Aetna PPO
  • Blue Cross PPO
  • some Blue Shield PPO plans, please call to confirm that we are in network providers. We do not accept Covered California Blue Shield policies at this time.
  • Cigna PPO
  • Healthnet PPO
  • Medicare

If we are not contracted with your provider or you do not have insurance, the minimum balance owed at the end of your first visit will be $135. Additional fees will apply for any procedures performed. Our office does not bill insurances that we are not contracted with. Patients are responsible for any balance that is not paid by their insurance.

Services that we are not submitting to insurance are due on the day of service. These include any cosmetic procedures (Botox, Juvederm, sclerotherapy, laser treatment, and chemical peels) or product purchase.
